Target Participants
Training officers and activity supervisors working at day activity centres (DAC) / day care centres for people with disabilities.
Therapy assistants, nursing assistants and healthcare attendants working in day rehabilitation centres, day care centres, nursing homes and sheltered homes etc.
Learning Objective
Upon completion of the course, participants will be able to:
· Explain the importance of having an individualised care planning for clients
. Develop individualised care, rehabilitation and training plans for their patients and clients
Course Outline
· The need to plan and do individualised planning
· The stages involved in developing an individualised care plan such as screening, reviewing and evaluating referrals with supervisors
· Skills needed in developing an individualised care plan
· Skills required in formulating a care plan in collaboration with other professionals, care staff and supervisors
· Common problems and issues in developing individualised care, rehabilitation and training plans when working with the elderly and people with special needs
· Prioritising problem areas to be addressed in the intervention process
Trainer
Mr Abhimanyau Pal is a trained occupational therapist. He has a master degree in health services management with a post-graduate qualification in business administration. Mr Pal has worked with children, adults as well as elderly population with various clinical conditions.
In the last 10 years, Mr Pal has provided the leadership, direction and motivation to drive and implement many rehabilitative programmes for elderly and people with disabilities in the community which includes post acute rehabilitation centres, paediatric rehabilitation centres, education programmes for students with disabilities in mainstream education system, social work and case management departments, day activity centre as well the sheltered employment programme.
